Checklist – Safety Inspection (Project Manager)

Must be completed during all site visits

ELECTRICAL safety

1.1 Are all extension cords and also cables free of visible damage?


1.2 Do all cables have a properly grounded plug (all three prongs are present in the plug)?


1.3 Are GFCI (Ground Fault) receptacles used on all power supply cords and tools?

PPE (Personal Protection Equipment)

2.1 Do all employees and subcontractors wear safety glasses?


2.2 Do employees working on or above the grating inside or working on scaffolding outside wear hard hats?


2.3 Do employees doing demolitions or working where there are high hazards wear hard hats?


2.4 Do all workers who transport tools or materials that weigh more than 55 pounds, or that may be exposed to other foot hazards, wear steel-toed boots?


2.5 Do all workers working in high traffic areas, whether in elevators or on foot, wear high visibility vests, jackets or other clothing?

General Conditions of the Site

3.1 Do all barricaded areas have sufficient fire extinguishers within the barricaded area?


3.2 Are all electrical cords taped or secured so they do not present a tripping hazard?


3.3 Is general cleanliness maintained at the site (including areas in front of and behind barricades)?


3.4 Are all construction entrances to barricaded areas marked with “Authorized Personnel Only” signs?


3.5 Are all work areas, tool storage, equipment, and break areas properly marked and barricaded?


3.6 Do all fenced areas have “no trespassing” signs posted on the perimeter fencing?

Motorized elevators

4.1 Do all motorized lift operators have a proper and current license (within 3 years) through Woods to operate that equipment?


4.2 Are all scissor lift chains in place?


4.3 Are all feet planted on the elevator platforms (ie, not on the railings)?


4.4 Lull and Forklift: are seat belts used?


4.5 Articulating boom and moving platforms attached to lifts: are slings used, positioned correctly and lashed correctly?


4.6 Do elevator operators outside the building wear DOT/Hi-Viz vest?

General Safety – Observations

5.1 Are all barricade doors closed and locked?


5.2 Are all the edges of the barricades aligned so there are no tripping hazards?


5.3 Do all emergency exit routes and doors have free and easy access and a clear path both to the door and from the door leading away from the building?


5.4 Are all ladders in use used safely? Set up correctly / Don’t stand on the top 2 steps / etc…?


5.5 Are all fire extinguishers, electrical panels, lifts/fire switches, and other emergency equipment free of obstructions and clearly accessible?


5.6 Do all workers who operate internal combustion equipment inside the building or other enclosed spaces use a carbon monoxide monitor?


5.7 Are all UNUSED propane containers stored outside the building?


5.8 Do workers involved in spraying operations wear approved respirators?


5.9 Are workers within 10 ft of leading edge (> 6 ft drop to next level) wearing harnesses and tethering to approved tie-down point (5400 lbs cap.)?


5.10 Do all containers containing chemicals have a label showing the contents of the container?


5.11 Are other trip/fall hazards observed inside or outside as you walk through the site (floor transitions, temporary floor changes, sidewalks, parking lots, etc.)?
