Checklist – World Health Organization Surgical Safety Checklist

The WHO Surgical Safety Checklist was developed after extensive consultation aiming to decrease errors and adverse events, and increase teamwork and communication in surgery. The surgical checklist has gone on to show significant reduction in both morbidity and mortality and is now used by a majority of surgical providers around the world. The checklist is not intended to be comprehensive. Additions and modifications to fit local practice are encouraged.

1.1 Has the patient confirmed his/her identity, site, procedure, and consent?

1.2. Is the anaesthesia machine and medication check complete?

1.3. Is the pulse oximeter on the patient and functioning?

1.4. Does the patient have a Known allergy?

1.5. Does the patient have a Difficult airway or aspiration risk?

1.6. Does the patient have a Risk of >500ml blood loss (7ml/kg in children)?

1.7. Confirm all team members have introduced themselves by name and role

1.8. Confirm the patient’s name, procedure, and where the incision will be made

1.9. Had antibiotic prophylaxis have been given within the last 60 minutes?

1.10. is the Team ready to handle the recovery and management of this patient?
