Checklist – Scaffold Safety

Use this template to inspect scaffold pole installation to ensure it is safe for use. Begin the checklist by capturing general information about the structure. Make sure the foundation and base of the scaffold are secure and check that the height is within safe dimensions. Inspect decks, braces, pipes, and standards for proper installation and detect any defects. Check the load capacity and access points to the scaffolding. Finally update the scaffolding tag and capture a photo

Base

1.1. Is the scaffold on a solid foundation?

1.2. Does each bracket have a base plate?

1.3. Are sole plates of the correct dimension being used?

Height

2.1. Are freestanding tower scaffolds stabilized with guy wires in accordance with regulatory requirements for their height?

2.2. Is a safety cage needed around the vertical ladder based on height based on regulatory requirements?

2.3. Is perimeter placed on work surfaces (permanent toeboard and temporary height) in accordance with regulatory requirements?

2.4. Is the height of the perimeter handrail with a center rail around all work platforms in accordance with regulatory requirements?

2.5 Are the rolling scaffold wheel brakes locked and outriggers extended to maintain a maximum height of 3 times the smallest dimension of the base?

2.6 Is the top rail based on height in accordance with regulatory requirements?

2.7 Are the mid rails installed approximately midway between the top rail and the deck surface?

Geometry

3.1 Are the standards vertical?

3.2. Are the stringers and crossbars horizontal?

3.3. Are the meetings in the standards staggered?

3.4. Are the joints in the stringers staggered?

3.5. Are the crossbars spaced correctly?

Platforms

4.1. Are the supports distributed correctly and evenly?

4.2. Are the overhangs the minimum standard size?

4.3. Does the plate not have a screeching sound?

4.4. Are lap boards safe?

4.5. Are the tables safe?

Bracing

5.1. Is stringer bracing installed on alternate pairs of standards for the full height of the scaffold?

5.2. Is façade bracing attached to each lift?

5.3. Is the façade bracing attached to the face of the scaffold at standard intervals?

5.4. Is the façade bracing placed at a standard angle?

Tubes and Accessories

6.1. Are used tubes and fittings in good condition?

6.2. Have the correct couplers been used?

6.3. Are all accessories tight enough?

6.4. Are retainer couplers installed as required?

Access

7.1 Is access provided to the full height of the scaffold?

7.2 Are the stairs set at a standard angle?

7.3 Is the head tied and the feet on a firm base?

7.4 Do the stairs rise to a standard height above the platforms?

Charging

8.1 Is the scaffold suitable for its intended use?

8.2 Are the loads on the scaffold evenly distributed?

8.3 Are stacked materials safe?

Ties

9.1 Are enough tie-downs used in accordance with safety regulations?

9.2 Do the ties attach with a double coupler?

9.3 Are the bindings set in the internal and external ledgers or standards?

Railings and skirting boards

10.1 Are the rails fixed at least 950mm above the platform?

10.2 Is the rail fixed a maximum of 470 mm above the skirting board?

10.3 Are toe boards installed on all decks and are they at least 150mm and secure?
